Hi, how are you? This is Eun-jin Kim, Doctor of Rosée Dermatologic Laser & Aesthetic Clinic, JoongAng Branch located in Banwoldang, Daegu, South Korea. Today, I’m going to explain about skin grafting for vitiligo.

​백반증의 치료 방법으로 국소 도포제(바르는약), 경구용약물(먹는약), 단파장 자외선 B(광선치료), 엑시머레이저, 피부이식수술 등이 있습니다.

 Treatment methods for vitiligo include topical application agents, oral medications, narrow band ultraviolet B (light therapy), excimer laser, and skin transplantation.

백반증이 발병한 경우 일차적으로는 비수술적 치료 방법으로 치료를 하게되며, 1년여정도 비수술적 치료를 꾸준히 하였음에도 불구하고 치료반응이 떨어지는 부위에는 수술적 치료를 하게 됩니다.

 When vitiligo develops, it is primarily treated with a non-surgical treatment method, and surgical treatment is performed on the area where the treatment response is poor despite continuous non-surgical treatment for about a year.

백반증의 수술적 치료방법에는 펀치이식술, 흡입물집이식술, 세포이식술 등의 방법이 있는데요, 현재 본원에서는 기존 수술방법들의 단점을 보완한, 마이크로 펀치를 이용한 전층피부이식술인 “ 미세피부전층이식술”을 시행하고 있습니다. 이는 백반증이 없는 정상 피부를 채취하여 백반증이 있는 부위에 이식을 하는 수술 방법입니다.

 Surgical treatment methods for vitiligo include punch grafting, suction blister grafting, and cellular grafting. In Rosee Dermatologic Clinic, micropunch grafting is performed, a full-thickness skin graft using micro-punch, that compensates for the shortcomings of existing surgical methods. This is a surgical method in which normal skin without vitiligo is taken and transplanted to the area with vitiligo.

Q1.수술은 어떤식으로 진행이 되나요?

 How is the surgery performed?

A1.일반적으로 귀 뒤의 정상 피부를 채취하여 백반증 병변이 있는 부위에 이식을 하게 됩니다.

 In general, normal skin behind the ear is taken and transplanted to the area with vitiligo lesions.

Q2.만약 귀 뒤에도 백반증이 있는 사람의 경우 수술을 할 수 없나요?

 Can't surgery be done for patients with vitiligo behind the ears?

A2.그렇지 않습니다. 주로 귀 뒤 피부를 공여부로 선택하지만, 귀 뒤에 백반증이 있는 분의 경우, 그 외의 다른 부위의 정상적인 피부에서 채취하여 수술하게 됩니다.

 Such patients can also have surgery. The skin behind the ear is mainly selected as the donor site, but in the case of a patient with vitiligo behind the ear, normal skin in another area is collected and operated.

​Q3.수술 후 주의사항으로는 어떤 것이 있나요?

 What are the precautions after surgery?

A3.수술 후 공여부(정상 피부를 채취하는 부위)와 수여부(정상 피부 이식을 받는 백반증 부위)에 밴드를 붙여 드리는데, 이 밴드를 7-14일여 정도는 떼어내지 않고 잘 붙여 두어야 합니다. 따라서 최소 2주 정도는 수영, 사우나 등은 피해주셔야 합니다. 또한 움직임이 많은 관절 부위를 수술할 경우 이식한 피부가 잘 안착하도록 1-2주 정도 과한 움직임을 피해주시는 것이 좋습니다.

 After surgery, bands are attached to the donor site (the site from which normal skin is collected) and the recipient site (the vitiligo site receiving normal skin transplantation), and these bands should be attached well for about 7-14 days without removing it. Therefore, you should avoid swimming and sauna for at least 2 weeks. Also, if you have operated on a joint with a lot of movement, it is recommended to avoid excessive movement for 1-2 weeks so that the transplanted skin can settle well.

Q4.수술할 때 통증은 어느정도인가요? 많이 아픈가요?

 How painful is the surgery?

A4.공여부(정상 피부를 채취하는 부위)와 수여부(정상 피부 이식을 받는 백반증 부위)에 국소 주사 마취후 시행하게 됩니다. 주사마취할 때 조금 따끔한 순간이 지나면 수술하는 동안의 통증은 없습니다.

 It is performed after local injection anesthesia into the donor and recipient sites. There is no pain during the operation after a slight tingling moment during injection anesthesia.

Q5.피부이식수술만 하면 백반증은 완치가 되나요?

 Can vitiligo patients be cured with just a skin graft?

A5.피부이식수술 후 엑시머 레이저 치료를 이어서 받은 경우 이식받은 병변의 치료 종결 확률이 높아집니다. 실제로 피부이식수술 후 엑시머 치료를 꾸준히 받았던 분들의 사례와 수술 후 개인적 사정으로 인해 엑시머 치료를 받지 못한 분의 사례를 비교해보았을 때, 수술 후 엑시머 치료를 꾸준히 받은 경우 해당병변 치료종결 가능성이 높았습니다.

 If excimer laser treatment is continued after skin graft surgery, the probability of terminating the treatment of the lesion increases. When comparing the cases of those who continued to receive excimer treatment after skin graft surgery and those who did not receive excimer treatment due to personal reasons, those who continued to receive excimer treatment after grafting were more likely to be cured of the transplanted lesion(end of treatment).
